Standard Security Life Insurance Company of New York is pleased to invite you to enroll in our claims portal. With your enrollment you will be able to see important information regarding your claim and make updates to your contact information at any time.

Services available through the portal, include:
            •	Electronic check copies with claim communications
            •	Current claim status
            •	If your claim is approved, your rate will display
            •	Updating contact information, including address, email address, phone numbers

PLEASE READ THE FOLLOWING TERMS AND CONDITIONS FOR USING THESE SERVICES.
            Review the terms and conditions regularly, as they may change at any time at our sole discretion.

By enrolling in the Portal:
            •	You acknowledge receipt of this notice concerning the operation of Standard Security’s portal, and that you will receive your claim notices electronically, and
            •	You consent to receive and send information electronically regarding your claim and to effect changes to your claim electronically, and
            •	You agree that we may contact you by email at the registered email solely for the purposes of providing you information regarding your claim and
            •	You acknowledge that your action in portal requesting claim changes constitutes your agreement to those changes
            •	You agree to provide and maintain a valid personal e-mail address within your portal account to facilitate our communication with you, and
            •	You agree to provide a valid mobile number within your portal account to facilitate any communication with you regarding your claim, and 
            •	You agree that we, and our affiliates may contact you at the registered mobile number via text, SMS, and calls which may include autodial and automated response, solely for the purposes of providing you with information regarding your claim
            •	You understand enrollment and participation is voluntary, and you may cancel at any time.

Enrollment: Standard Security’s portal is a voluntary and optional services; you may discontinue your enrollment at any time.

System requirements: To participate in Standard Security portal on the internet, you will need a computer with a web browser that supports at least 128 bit encryption. By enrolling in the portal you acknowledge that you have the ability to receive and read e-mail as well as Adobe Portable Document Format (PDF) attachments.

Paper copies: You may request a paper copy of your claim, as well as any portal transaction document by contacting us at 800-477-0087 or writing us at Standard Security Life Insurance Company, P.O. Box 25339, Farmington, New York 14425.

Update contact information: You may update your contact information in the following ways:
            •	by making the change yourself directly within your portal account
            •	by writing to us at Standard Security Life Insurance Company, Standard Security Life Insurance Company, P.O. Box 25339, Farmington, New York 14425

Cancellation: You may cancel your enrollment in Standard Security portal at any time; however, doing so will not change (1) your obligations under any claim including, but not limited to, providing timely remittance of requested information for any claim or (2) the legal effectiveness, validity, or enforceability of electronic records that were made available to you prior to the implementation of your cancellation. If you cancel your enrollment in Standard Security will deliver all notices to you using United States mail or via email upon request.

To cancel your enrollment in the portal, you may contact us in the following ways:

Note: Cancelling your enrollment may result in unavoidable processing delays of claim. Please allow up to two (2) weeks for cancellation to take effect.
